Otitis externa is a very common problem in daily practice. Proper management of otitis externa involves investigating, identifying and treating the causes and factors involved in its occurrence and maintenance.

Primary causes can cause otitis on their own, while secondary causes can cause otitis when acting on an 'abnormal' ear and are the most easily identifiable and treatable. Predisposing factors are those that increase the likelihood of developing otitis externa by modifying the internal environment of the ear, while perpetuating factors correspond to changes in the ear due to the otitis itself.

Hypersensitivity (atopy, food intolerance and contact dermatitis) is one of the main primary causes of otitis externa. The incidence of otitis externa in dogs with atopy is estimated to be 55%; and in some of these cases, otitis externa may be the only sign. Some studies indicate that dogs with atopic otitis are 5 times more likely to have foul-smelling discharge than dogs without otitis1 .

Excessive moisture in the ear canal is one of the predisposing factors for otitis externa. Environmental changes, such as increased temperature or humidity, or water entering the canal (e.g. in swimming dogs), can affect the condition of the external auditory canal.
On the other hand, cleaning the ear canal with solutions that increase humidity or the use of inappropriate solutions can alter the condition of the ear canal, especially in sensitive skin.
